Please bring the following items with you to your appointment:

— Insurance card
— Physician’s Order (Prescription)
— Prior Imaging / X-ray / Mammogram Examination
— Referral forms from Insurance Company

• Please be sure any precertification requirements of your healthplan are met prior to your appointment.
  
• Please arrive at least 30 minutes prior to your appointment.
  
• Exam results will be sent to your physician within 48 hours.
  
• If you need to check out your x-rays, please call us 24 hours in advance.
  
• If you have any questions about preparation or your exam, please contact us a the appropriate number.
  

• BARIUM ENEMA

• CT ABDOMEN and/or PELVIS

• CT EXAM (REQUIRING I.V. CONTRAST)

• ESOPHOGRAM / BARIUM SWALLOW

• IVP

• MAMMOGRAM

• MRI

• ULTRASOUND (ABDOMEN / GALLBLADDER / KIDNEY / AORTA)

BARIUM ENEMA:

1. Pick up "X-Prep Kit #1" from our office.
   
2. Follow instructions.
3. REQUIRES 24 HOURS.

CT ABDOMEN and/or PELVIS:

1. Pick up oral contrast bottles from our office.
2. Drink one bottle four hours prior to the exam, and another one hour prior to the exam.
3. Except for the contrast, do NOT eat or drink anything after midnight, or for at least four hours prior to the exam.

MAMMOGRAM:

Do NOT use deodorant, powder, or perfume prior to exam.

MRI:

1. Assure that you do NOT have a pacemaker or brain aneurysm clip.
2. Notify staff if you have any metal anywhere in your body.

Do NOT eat or drink anything aftermidnight or for at least six hours prior to the exam.
