Exam Referrals

Most procedures done by RRA require a referral from a primary care or specialty physician, especially when a health plan will be paying for services. If a patient is paying his/her own bill for services, such as a routine mammogram or bone densitometry exam, a referral may not be necessary and the patient can simply make an appointment with us.

Exam Referral Forms
Area physicians have been provided a supply of Exam Referral Forms to indicate the specific screening, diagnosis and/or procedures that are being ordered. These forms can be faxed or brought to our locations. If a referring physician's office does not have the forms they can be downloaded by clicking this Exam Referral Form link.
